This activity features an engaging multidisciplinary faculty discussion about special considerations in the care of older adults with T2DM. Many older adults with T2DM are experiencing negative health consequences that could be prevented or improved with optimal evidence-based care. This is a heterogeneous population that includes individuals residing independently in communities, in assisted living, and in skilled nursing facilities. They can be fit and healthy or frail with numerous comorbidities. Thus, management of diabetes in older adults should be individualized, taking into account these variables. All members of the care team, including physicians, NPs, PAs, pharmacists, and nurses have a role to play in improving outcomes in their older adult patients.
This activity is intended for senior care clinicians, including physicians, NPs, PAs, pharmacists, nurses, and others who encounter older adults with T2DM.
Upon completion of this educational activity, the participant should be able to:
- Outline team-based strategies to maximize safety and efficacy with respect to glycemic control for T2DM in older adult patients.
- Discuss the effects of various diabetes therapies on the risk for hypoglycemia.
- Describe multidisciplinary strategies to prevent fluid/electrolyte imbalances and drug interactions.
- Summarize data regarding evolving science and emerging therapies for the treatment of T2DM in older adults.
